The TULIPS Template

The TULIPS template is designed to help you prepare your End of Life Plan rather than as a replacement for it. Think of it as collecting all your most important thoughts on a single page.
There are no rules but one approach could be to think of the template as ‘prepping the soil’ before planting any TULIPS by turning on the TAP:
T: Topic- the main areas to consider
A: Action - things you have already done or plan to do
P: People - those who are best placed to know what you want and/or that you have nominated to carry out a particular role.
You can choose just one of these aspects, all of them or none. Remember, there is no ‘right’ or ‘wrong’ here, only what’s right for you!
